In order to get the most value out of your membership it is important that you clarify what your longer term goals are as well as the more immediate ones.  It is also very helpful  to identify what areas in your lifestyle you have the most interest in working on to  improve upon to support you achieving the goals you have identified.

This Welcome Questionnaire will help you clarify  your goals and identify the areas in your lifestyle  that you wish to give more  attention to. Once you have gone through this questionnaire  then you will know what resources and support you should start with at this time rather than trying to tackle too many steps at one time.

Failing to plan is planning to fail so let’s start with you starting to build your plan with the help of this questionnaire.
